A Spa for Paws Grooming Release Form

VACINATIONS: A Spa for Paws (located at Red River K9 ll LLC) requires Rabies, Distemper, and Bordetella vaccinations.

COAT CONDITION: I understand that A Spa for Paws puts my dog’s safety and comfort above all else. In the event that my dog’s coat is matted, the groomer may have to shave the matts out rather than perform a painful dematting procedure. If my pet is severely matted, there is an increased risk for clipper burn or cuts to occur. All attempts will be made to prevent this, however in many extreme matt conditions, it is unavoidable. I also understand that matted dogs take additional time to groom so there will be an additional fee added on to the regular grooming price if my pet’s coat is matted. *

FLEA POLICY: Fleas are not only a nuisance to humans and their pets, but can cause medical problems in pets including flea allergy dermatitis, tapeworms, hair loss, and secondary skin irritations. A Spa for Paws has a strict policy that we do not accept pets with fleas. If fleas are found on your dog during the grooming process, an automatic charge of $20.00 or more will incur for flea treatment shampoo and facility sanitization. I will not hold A Spa for Paws responsible for any reactions to the flea treatment. *

HEALTH: I understand that grooming can be stressful to some dogs and I will inform the groomer if my dog has any medical conditions, stress related, or aggressive issues prior to grooming. Grooming can also expose a hidden medical condition or aggravate a current one. This can happen during or after the grooming process. I understand A Spa for Paws has the right to stop the grooming process at any time for the safety and well-being of your dog or ourselves (prorated fees apply). *

EMERGENCIES: Although accidents are very rare, there is always a risk when dealing with animals. Grooming equipment is sharp, and although A Spa for Paws uses extreme caution and care in all situations, possible problems could occur including cuts, nicks, scratches, quicking of nails, etc. In most cases this can happen when an animal is wiggling or moving around. Your pet’s safety and comfort is our number one priority. In the event of an emergency, I authorize this establishment to immediately seek professional veterinarian attention for my pet (at my expense). I understand that all attempts will be made to contact me in the event of an emergency. I will not hold A Spa for Paws responsible for any accident or injury to my pet. *

CANCELATIONS: The staff at A Spa for Paws would greatly appreciate any cancellations be made at least 24 hours before your scheduled appointment time. No shows or appointments canceled less than 24 hours before the scheduled time may incur a cancellation fee of $25.00. *

LATE ARRIVALS: Please call us if you know that you are going to be late for your appointment. We will do everything we can to still be able to give you the service you have booked, however if you are more than 15 minutes late we may not be able to complete your service and still be on time for our next pet. If so we will need to reschedule your appointment and it will be considered a late-cancel and subject to cancellation fees. *

LATE PICK-UP: When dropping off your dog we will give a time frame of when we anticipate completion of services. Please keep in mind that grooming can be a very time consuming and unpredictable process depending upon the dog’s behavior and the condition of the coat, as well as when clients arrive for their appointments. We will call or text you 30 minutes before your pet is finished. Any pet picked up over one hour after we have called/texted will be charged $10. We will not be held responsible for anything that happens while your pet is waiting to be picked up. If you do not pickup your dog by the time we close for the day he/she will spend the night and boarding fees of $45 may apply. *

PAYMENT: At A Spa for Paws we accept cash or check. For checks there is a $35.00 return check fee. Payment is expected at time of service. *

PHOTOGRAPHS : A Spa for Paws Pet Grooming may take pictures of your pet before, during, and after grooming for our files ,website, advertising and education. *

By signing below, you indicate that you understand and agree to our terms of service; that you understand and agree to release and hold harmless A Spa for Paws Pet Grooming, its owners, employees and affiliates from and against all liabilities, expenses, damages, and costs (including attorney fees) resulting from any service provided or injury (including death) to your dog(s) while in our care or afterward.
