In admitting my dog for services I understand that Red River K9 II LLC has relied upon my representation that my dog is in good health, has not injured/shown aggression/threatening behavior to any persons or dogs, and does not escape fences. *

I understand that Red River K9 II LLC's owners/staff/volunteers will not be liable, financially or otherwise, for injuries to my dog(s), myself/whomever may be with me, or any property of mine while my dog(s) is/are participating in services provided by Red River K9 II LLC. I hereby release Red River K9 II LLC of any liability of any kind arising from myself/whomever may be with me or my dog(s) participation in any and all services provided by Red River K9 II LLC or while on the premises of E401 Borley Lane, Luxemburg, WI 54217. *

I understand that my dog must be free of external parasites (fleas and ticks) and be on a flea preventative from May to November. I understand that my dog may still get fleas and/or ticks even if on a preventative. *

I further accept the responsibility of keeping my dog(s) current on Rabies, Distemper and Bordetella vaccinations and understand that despite my dog(s) being current on the Bordetella vaccine, it is still possible that my dog(s) could contract kennel cough. *

I understand that I am solely responsible, financially or otherwise, for any harm or damage caused by my dog(s) while my dog(s) is attending any services provided by Red River K9 II LLC and that services going forward may be refused if Red River K9 II LLC deems that my dog(s) is an unsuitable fit. *

I hereby agree to allow Red River K9 II LLC to take photographs/videos of me/my dog(s) to use in print form or otherwise for publication and/or promotion. *

I understand that if my dog(s) is/are not picked up on time or by a date otherwise specified in a separate agreement, I hereby authorize Red River K9 II LLC to take whatever action is deemed necessary for the continuing care of my dog(s). I will pay Red River K9 II LLC the cost of any such continuing care upon demand. I further understand that if I do not pick up my dog(s) Red River K9 II LLC will proceed according to the guidelines provided by Wisconsin State Legislature regarding abandoned animals. I also acknowledge that I will be fully responsible for all attorned fees and associated costs if I abandon my dog(s). *

Payment is required prior to service. Payment can be made by cash, check or card (service fee). Checks can be made to Red River K9 II LLC Leashes are required to be utilized while in the parking lots. I agree that if I choose to not leash my dog(s) while in the parking lot at Red River K9 II LLC located at E401 Borley Lane Luxemburg, WI 54217 Red River K9 II LLC and their employees/volunteers will not be held responsible if my dog(s) get injured, deceased or runs-off while outside of the building/vehicle. *
